Brief Answer:
Lumbosacral radiculopathy/MRI L-S Spine/Antineuralgic medicines/Exercises

Detailed Answer:
Hi,
Thanks for being in follow-up and providing further details.

Considering your clinical details,I am of the impression that you are probably having lumbo-sacral radiculopathy.
I suggest you to get a MRI of Lumbo-sacral spine region.

For the treatment part,I suggest you to start antineuralgic medicines like pregabalin, gabapentin or amitriptyline. In addition, you should be on methylcobalamin and calcium/vitamin D supplementation.

and lastly,I suggest you to consult an expert physiotherapist regarding the training for spinal exercises.

Hope that I have answered your query. If you have some further questions,I shall be glad to answer else please close the thread, rate it and write a review.

Regards
Dr.Ajay Panwar,
