With history of use of ACE inhibitors the red rash is quite likely the connection to the meds. 4 to 7 % people get this rash and itching in it (also called urticaria). You may stop the medication after consulting with your doctor and replace it with a better alternative.
As far as the hands and legs puffiness is concerned, so edema is the likely possibility of it and to assess, first step would be to rule out cardiac perfusion studies. I hope you understand but the comorbidities you have makes it difficult to diagnose this itching online. You need a proper dermatological exam. Did you try antihistamines, topical hydrocortisone creams etc for the rash.
Also i mentioned one of your antihypertensives may cause this rash too. That needed to be replaced with alternative. Also it takes some time to get the drug out of system completely.
But best course here is to start with a consult to a dermatologist and let them help you in this regard.
