Brief Answer:
Allergic reaction
Detailed Answer:
Hello
Thanks for the query and photograph.
You have developed scrotal dermatitis.
It is an allergic reaction.
Ketoconazole is an antifungal and would not be helpful.
I treat such cases with a mild steroid cream like cortisone to be applied twice daily for a week.
This should help you.
Let me know if you have any other doubts.
Thank you.

Detailed Answer:
Hello
Thanks for the follow up.
Both scrotal dermatitis and fungal infection looks similar.
We do lab tests like KOH mount for fungal hyphae.
Since that is not possible in online queries it is better to go ahead with the treatment of more common condition that is fungal infection.
If there is no response diagnosis as well as treatment changes.
Hope that helps you.
Let me know if you have any other doubts.
Thanks
