Question: I have to figure out what is going on and at this point do not know which specialist to see. I am 36 yr old male, 5 ft 10 in 185 lbs. and have had worsening fatigue over the last 18 months. I have had a few chest and head CT scans, blood work, pulmonary function test, echocardiogram, nuclear stress test, upper endoscopy, mri of abdomen, emg on muscles. So far it appears I have some mild lung scarring/asthma that they say could be c, ongenital but would not be causing my symptoms. Initially said my pulmonary pressure was mildly high and heart slightly larger than normal. Had another echo and supposedly all is now normal?? Also hematocrit was high so I had to go to hematologist and got it down to 44 from 52 thru multiple blood draws over 3 MO span. Cholesterol sborderline high and testosterone was around 280. I currently take Prime Male supplements for Low T, fish oil, coq10.
My symptoms are extreme fatigue as I can no longer exercise comfortably. My heart sometimes fluutters and I cough to covrt it and stay on track Legs cramp after about 5 min of jogging. Leg tightness and stiffness/soreness/aching, hair thinning on ankles, pressure in chest and heavy head /dizziness on left side. My legs and feet twitch and bottom of feet cramp frequently. I just feel heavy all the time and don't know where to turn, my wife and I have 3 children and t his is affecting my lifas welll as marriage. Please let me know which direction to head!

Hello,
Thanks for posting in HCM.
I have gone through your query and understood your question.

I would like to know more about certain things.
1. How is your sleep?
2. Is it disturbed? Or sound sleep?
3. Do you get up tired or drowsy or having headache?
4. Ask your wife whether you snore loudly in night which disturbs her sleep.
5. Do you feel sleepy throughout the day?
I am asking all this question because seeing your height weight and age you might have what we call as obstructive sleep apnea.

It is an common cause of problems because of disturbed sleep due to obstruction causing breathing difficulty in sleep which causes tiredness fatigue throughout the day.
For diagnosing this problem, a test called as sleep study (polysomnography) is done. Your sleep pattern is studied for 1 night and read the next day.

As your all tests are normal and not able to get diagnosis, this is the next thing to do as it is usually ignored to be a problem. Ask your doctors about this and get it tested.
If it is indeed the problem then sleeping with CPAP or BiPap will help you considerably.

I hope I have answered your query well and given you helpful advice.
If you have any questions kindly ask me back.
Thank you
