Brief Answer:
TMT/Stress Echo/X-ray chest required for diagnosis
Detailed Answer:
Hi there,
Thanks for posting your query on this forum.
The presentation of symptoms suggest an angina attack. ECG and
Troponin I (I hope you meant Trop I instead of Trip in I) done after the episode are usually negative and do NOT exclude underlying
ischemic heart disease.
You need to get TMT/Stress ECHO and 2-D Echo to rule out/diagnose underlying heart disease. As such, you have a risk factor in the form of
overweight.
I presume that your blood pressure is normal as you have not made any mention about it.
However, the other possibilities can be collection of fluid (due to infection) in the
Pleural cavity ( a bag-like structure surrounding each lung; impending
heart failure (in view of breathlessness on minimal exertion); or,
cervical spondylosis with bulging discs compressing the nerves of left arm and chest.
Hence, additional tests required to exclude/diagnose these possibilities are :
* MRI of cervical spine
* Chest X-ray
* Blood sugar and Thyroid hormone assay to rule out Diabetes and
under-active thyroid, respectively.
Consult your doctor and apprise him of my opinion. I am certain that he will agree with me; order required tests, and- based on the reports- will arrive at a certain diagnosis and prescribe suitable treatment as advised.
Hope I have addressed your concerns to your complete satisfaction and you find my response helpful and informative. I will be happy to answer further queries, if any.
Take care,
Col (Dr.) Rakesh Karanwal
