Question: Ai had a tkr 3 yrs ago in XXXXXXX I have pain and difficulty walking. My OP said pain was coming from my hip. I went to another OS for second opinion and he said knee could be unstable. I can walk up and down stairs, ride a bike drive and sleep without problems. I just can not walk without pain. It's getting worse instead of better. I did get a shot in right hip last year under XXXXXXX and for 2 days I had no pain. However it didn't last. I'm at my wits end.

I read your question and i understand your concern. Age of 74 with history of knee replacement surgeries , opinions of both the doctors are right fromt heir own perspective. Age related articular degeneration and over burdening your knees with the work beyond the scope of geriatric limits may be the reason.

As far as steroid injections are concerned, they can be repeated and a stronger and more potent analgesics can be advised. Physiotherapy along with other modalities like acupressures, acupuncture and biofeedback can be continued meanwhile. If you have a weight problem that needs to be shed as weight gain puts extra articular burden exacerbating the pain. Gait cycle training is advised as knee instability may cause all these problems.

Nutshell, age limitations make the pain management conservatively the likely and feasible approach for the management. Meet a pain medicine experts and let them devise a plan for the pain relief.

Just a follow up. I don't have a weight problem. I never have had one. I will follow up again with my OS. I have an appointment in May. My follow up question is, should I have rested after getting steroidinjection? I felt so good, no pain, that I immediately started doing gardening, driving next day and sitting for 4 hours playing bridge. I think I did too much too soon. Should I have rested before doing any activies?

You narrated the main culprit right now. After epidural steroid or any other joint steroid injections minimum of one week of complete bed rest is the necessary and mandatory component. Had it been followed you would not have a problem at all.

You may opt for steroid injection again and this time with do as directed policy.
