Brief Answer:
It can be gall bladder problem.

Detailed Answer:
Hello,
I have studied your case. Two type of possibilities are there.
1. it can be gall bladder problem with pain radiating to shoulder.
2. it can be cuff problem in shoulder which is common in diabetic.
you need to get a MRI of the shoulder and ultrasound of the abdomen to confirm the diagnosis. If there is gall bladder problem then you might need cholecystectomy.
I hope this answer will be useful for you. let me know if there is any other followup questions. thanks
