If the Hashimoto's is not the cause of the symptoms then it is possible that the cause could be a problem with his heart or some sort of yet undiagnosed infectious disease. I suggest a referral to a cardiologist first and if that does not turn up a cause then a referral to an infectious disease specialist. Heart disease can sometimes present as nausea instead of chest pain and swelling of the limbs and fatigue is also common with heart failure. Likewise an infection could also cause most of his symptoms. Sometimes infectious diseases are hard to identify and seeing an infectious disease specialist could help sort this out.
