Question: I have started to have heart palpitations.i had a spell a few weeks ago where my heart sped up and was very irregular for a few hours speeding up and down and fluttering..since then I have times of skipped beats and the palpitations are always accompanied by bloating and belching of gas..what might be the cause

Regarding your concern, I would like to explain that those symptomatic heart palpitations are very likely to be caused by a cardiac arrhythmia, most probably atrial fibrillation.

Coming to this point, the best strategy for directly identifying potential arrhythmic event is to perform an ambulatory 24 to 48 hour ECG monitoring (Holter).

In such way, atrial fibrillation and any other potential cardiac arrhythmia would be properly diagnosed (also a quantitative study of arrhythmia would be possible).

Also, a careful physical examination, a cardiac ultrasound and some additional lab tests as follows:

- complete blood count (for investigating anemia),
- thyroid hormone levels (for possible dysfunction),
- blood electrolytes level,
- renal function tests,
- fasting blood glucose and HbA1c (if necessary),
- chest X ray film, etc,

would be necessary to investigate potential causes of such an arrhythmia.

Sometimes, even a digestive disorder (such as hiatal hernia, GERD) may lead to cardiac rhythm disturbances. In such case, a fibrogastroscopy would help to confirm a potential underlying disorder.
