What Causes Small Bumps On Forehead?

Question: hello doctor, I am suffering from small bumps on my entire forehead from almost month. I was using tretinol in night but no result, I have done chemical peel also but still there is no improvement. can please suggest any ointment which I use and also I would like to mentioned that same kind of small bump now started in my neck, back n front.

I am Dr, Kakkar. I have gone through your query and I have understood it. I have also seen the Images.

This is classical comedonal acne. I can see a lot of whiteheads also known as closed Comedones

Tretinoin is an excellent comedolytic agent. The strength recommended for face is 0.025% once daily at night.

Chemical peeling, specifically Salicylic acid peel also works well for acne as well as comedones. You may take a session once every 2 weeks but never while you are using tretinoin because it might burn the skin. A gap of at least 1 week after stopping tretinoin and salicylic acid peeling is adequate/safe interval.

However, If I was the treating doctor I would have advised you to continue with tretinoin once daily. Comedones are slow to respond but they ultimately would reduce in size and disappear.

Since you have comedones over chest and neck as well therefore, I would have also added either oral Vitamin A 25000 IU twice daily after meals Or oral retinoids e.g Isotretinoin. These are excellent oral medications for acne, and work for all grades of acne, however, you need to get a written prescription for these from a dermatologist

Another option is to go for comedonal extraction from a dermatologist. This can be combine with topical and oral treatments, mentioned above

Hi.

You can add benzoyl peroxide 2.5% gel for day time use while tretinoin is for night application. Benzoyl peroxide is also coemdolytic.

Never use both of these together/simultaneously because they inactivate each other.

Hello doctor,

can please specify more with Tretinoin 0.025% cream because which is i am using is in gel form and as of i check there are various more cream with same 0.0255 so please tel me which brand i have to purchase for better result.

Hi,

thanks a lot for the answer but which i am using its Retino-AC 0.025% marketed by XXXXXXX and XXXXXXX and its gel form, so its give same result or should i purchased the cream base.

Hi.

You can use either of these. The difference being that the one you are using contains clindamycin in addition to tretinoin. Clindamycin is an antibacterial and its main use is for inflammatory acne. Clindamycin has no effect on comedonal acne. However, you may continue with the gel that you are using because it contains tretinoin, nevertheless.
