Hello,
Thank you for posting on HCM.
I appreciate your concern regarding your skin condition and will make sure your problem gets solved in a rightful way.
After keen review of your history and photograph, I think you might have got some fungal infection over penis (Candidial Balanoposthitis or Tinea) instead of Herpes progentialis but still its difficult to say anything without actual examination.
As you are already having jockitch which is a fungal infection, it is very likely that it might have spread to your genitals also. Moreover this infection is more common in uncircumcised men.Poor hygiene and excessive use of harsh soap can also can predispose to the infection.
I would insist you to post clinical pictures with better clarity (Also one picture where the foreskin is completely retracted !) and mention the following points of history on follow up:
1. Apart from whitish discharge collection any associated symptoms like itching,burning sensation while urination, ulcers over genitals, lumps in groin area (enlarged lymph nodes) etc
2 Sexual exposure history
3. History of similar complain in past
4 History of any complain in your partner
5. Are you suffering from diabetes or any medical condition
6. Any other significant points which you think can help.
Generally in my patients of Candidial balanoposthitis, I suggest a short course of oral antifungal like fluconazole 150 mg once daily for 10-15 days.
Apply topical antifungal cream like sertaconazole twice a day over affected area for few weeks.
Use ketoconazole soap for rinsing the area and use antifungal dusting powder to keep the area dry.
Take antihistaminics for itching as required.
Maintain proper hygiene and Use cotton undergarments and avoid too tight undergarments.
