This might be what we call paraphimosis. This occurs when the foreskin in persons who have never been circumcised constricts the glans penis and leads to inflammation, pains, and urination difficulties.
If you have never been circumcised, I will suggest that you get this looked into by your doctor. Ou may need to get a release or maybe be considered for some form of circumcision to address this is if you have never done so before.
