Question: Doctor, I have inflamed skin on my penis including constriction at the end of the skin... very sore and sensitive... cannot even wear underwear

This might be what we call paraphimosis. This occurs when the foreskin in persons who have never been circumcised constricts the glans penis and leads to inflammation, pains, and urination difficulties.

If you have never been circumcised, I will suggest that you get this looked into by your doctor. Ou may need to get a release or maybe be considered for some form of circumcision to address this is if you have never done so before.

I will not encourage that...Ibuprofen/Tylenol orally for pain would do!

I cannot recommend a topical agent without a closer evaluation. For now, I recommend that you go ahead and take a pain medication to reduce the swelling and pains. When reviewed by a doctor, the decision on a topical agent or another procedure would be discussed with you.

Am sorry to learn you are not happy with the response. The truth is what you described looks more of a surgical problem than a medical problem. If I could determine that it is an infection, I would be providing other options. But because I would not want to mess up anything and rather want that you get a prompt attention for evaluation and treatment, managing your pain and proposing you see a doctor is the bets option for you at this time. You may not be satisfied but providing information to satisfy you and not help the exact disease condition is wrong.

One last option is to send an image for review. If I get definitive clues, I would make further suggestions. Otherwise getting it examined is the next best thing to do.
