What Causes Soreness In Testis?

Question: Three or four years ago I had a testicular trauma. The left testicle would get sore from time to time and sometimes I would hear gurgling and see sperm come out with urine when I urinated. The testi hangs lower then the other and is a little smaller than the other. I had a physical three days ago and the PA said everything seemed fine except for tenderness in the left testicle. He scheduled me to see a urologist in two weeks from when i saw him. Is there something wrong or am i worried for nothing? Also if the testicle was torn or dead wouldn't i have bigger problems right now and wouldn't the physical I had have shown anything? Also would a doctor be able to tell if a testicle is dislocated by regular physical examination? And how long could a ruptured or torn testicle stay in scrotum without getting infection like gangrene?

Thanks for your query,based on the facts that you have posted it appears that you have soreness in Lt testicles from time to time.

At the outset I would state that the injury to Testicle that you sustained 4 years back will not have any effect after such a long time .Neither your testicle is torned nor damaged .It is anxiety that is bothering you.Torned or damaged testicle would not survive for longer than a week and would become gangrenous within few days.

In every individual one testicle is always placed at lower level than other and the size of the both the testicle is never similar. It is natural and normal to have one testis bigger than its counterpart and has no significance at all.

Soreness in testicle could be due to mild Varicocele.This can be confirmed or ruled out by a following tests.
1) Ultrasound scanning of Scrotum.
2) Colour Doppler study of Scrotum.

Well, my left testicle also sits in a different direction from the other and it feels looser to me. Also I have to sit a certain way or it feels like i'm going to sit on it. I had a physical a few days ago and the doctor said everything seemed fine. He was a PA not an MD. He referred me to see a urologist in two weeks. Would he have known if something wasn't right? How fast does gangrene work out of curiosity?

Thanks for writing again.
The testis is always free and XXXXXXX for upward and downward movements so as to be placed in scrotal sac depending upon the atmospheric temperature for effective production of sperms..It does not make any difference in which direction it is placed in scrotal sac as long as it does not undergo torsion .
It will become gangrenous within few hours of torsion due to occlusion of blood supply (Ischaemic Necrosis)resulting from twisting of blood vessels .
