Question: hi
62 Y.O. recently developed spontaneous nose bleeds. She has just been flying an couple days ago. She is a prior NICU nurse.
BP was 169/84 HR 71 strong at the time of last onset. Then dropped to about 145 in 30 minutes and 1hour later.
It happened twice in two days. She reports no Hx of nosebleed. There may be some genetic predisposition. No lesions or bruises. Cap refill was a bit slow at full 2 seconds. No pulse-Ox available. She packs it and pinches for about 30 minutes to stop it. She applied a nasal spray both times on advice of a nurse friend. Blood loss is less than 10cc.

Brief Answer:
accelerated hypertension/ atrophic rhintis- needs evaluation.

Detailed Answer:

Hi,

I had gone through your question and understand your concerns that you are having recurrent nose bleeds.

As per your clinical details, it appears to be your nose bleeds could be due to accelerated hypertension or self picking of dried nasal crusts ( most common cause of nasal bleeding in elderly persons).

Normally, nose bleeds are controlled by decongestants (oral +nasal), these are contraindicated in hypertension.

I suggest you to visit an ENT surgeon and give history of medication (aspirin), detailed ent examination (bleeding polypus or drying of crusts secondary to atrophic rhinitis), blood tests (anemia). Depending on the examination reports further treatment can be started.

Hope this answers your question, if you have additional questions or follow up questions then please do not hesitate in writing to us. I will be happy to answer your questions.

Wishing you good health.
