Detailed Answer:
Hello!
Welcome and thank you for asking on HCM!
I understand your concern, and would like to explain that, based on your pain characteristics (stabbing pain modulated by posture and deep breaths), it doesn’t seem to be cardiac related.
A cardiac ischemic pain isn’t like this. So relax! You don’t need to think first of a cardiologist as your pain is most probable to be of extra cardiac origin.
I recommend you to consult with an internist, as a comprehensive differential diagnosis should be done.
Several causes like a musculoskeletal disorder, a backbone degenerative disorder, a pleural affection (inflammation or adherence), esophageal spasms, pulmonary micro embolisms, or even shingles (herpes zoster), etc should be sought.
A careful physical exam, a resting ECG, chest X ray study, several lab tests, like a complete blood count, PCR, D dimer, LDH, AST & ALT, etc. would be helpful for the differential diagnosis.
If suspicions are raised, then chest angio CT, cardiac ultrasound (for measuring pulmonary artery pressure and investigating right heart chambers), or lower limbs Doppler ultrasound (for investigating possible vein disorders as a source of pulmonary emboli), etc should be done.
You need to discuss with your attending doctor about the above mentioned issues.
Hope to have been helpful to you!
Feel free to ask me whenever you need! Greetings! Dr. Iliri
