Brief Answer:
bacterial infection with Gastrointestinal involvement mostly.

Detailed Answer:
Respected XXXX, Hi, good evening.

* Considering all the data given herewith narrate Gastrointestinal infection due to some bacterial involvement.

* My clinical impression is Enteric Fever as the first possibility which is caused by
Salmonella typhi bacteria invasion through gastrointestinal tract;
confirmation of which requires '' Serum-WIDAL'' test and Blood Culture, sensitivity in Laboratory.

* Second possibility of H.Pylori infection for which confirmation report of biopsy is awaited, so its comments will be in relation to the result of that.

* Till that period, don`t panic, relax, follow certain measures as below:

- drink plenty of liquids preferably boiled water, coconut water, lemon juice,
green tea, coffee, butter milk, home made juices to maintain hydrations.

- take light diet in form of rice, yogurt, khichdi, boiled milk, mung.

- avoid all oily / spicy / nonveg stuffs / excess ghee / cheese / paneer /
junk foods / commercial beverages / biscuits / ice creams etc .

- Tab. Pantoprazole 20mg + Domperidone 10mg (eg. PAN-D) morning & evening an hour before meals on an empty stomach,

Tab. Acetaminophen (325)mg 8 hourly after meals,
Tab. Cefixime (200)mg 12 hourly after meals,
Syrup Digene gel 2 table spoonful every 6 hourly for minimum 3 -5 days.

- continue thyroid medicines. ( advisable to check Thyroid profile with S.Widal test in the laboratory to evaluate any thyroid level imbalances giving symptoms, if not done in past 3months)

- avoid smoking/alcohol totally

- avoid stress/anxiety
- try to have a sound sleep of 6-7 hours
- avoid prolong fasting/overeating
- do some light exercises, deep breathing exercises, YOGA at home
- maintain a chart of your BP, Temperature, Weight till total recovery

- chew your food thoroughly
- try to control stress/anger/emotions as its detrimental for flare up of all ulcer symptoms
- avoid sleeping for 2 hours at least after meals

* These are definitely going to help you at good extent.
