Question: Hello. I am 54 year old woman. I have RA fibro also had fascia release surgery to help pain. Only now to be worse I am on methotrexate gabpentine dulexatine 4 7.5 Percocet daily. I had a tooth break. I could not address it. Now my whole mouth rotted in incredible pain. My joints so bad can barely walk. Still have to work as I am a single mom. I lost my pain meds this month. I do understand they are regulated. But I don't know if my stomach cramps and black diarrhea is from stopping med abruptly both pain clinic and dr say no and tough it out Perhaps I m suffering because of high pain making me sick. Last month I went to pcp with cysts underarm. Agin dismissed. I m at a loss There is so much going on and I'm rotated to so many Drs so no one has to be responsible or h to address me. so I sit here in pain my joints are getting worse each day Teeth problems now 24/7 nausea and black diarrhea. Is there anything I can do besides buck up
Thank you XXXXXXX

Also the past few days started a mild cough and mild chest tightening. I'm a non smoker. Not sure if is all a part of it. But the heat index has been over 100. Not sure if it means anything

Brief Answer:
so many things, so many possibilities.

Detailed Answer:
Ok, let us start with the most common possibilities. Tooth ache, cysts underarm, cough and lung symptoms are all signs of infection. These of course always make RA much worse, but rather more to the point is that in RA and especially on methotrexate someone is more susceptible to infections producing death. They need to be taken care of. Unfortunately various circumstances have caused acute care to be addressed predominantly by acute care facilities rather than like, doctors. So, the stupid fact is that the new normal is to have acute medicine at ER-like facilities.

Ok, now the bad part.

Methotrexate often causes scar tissue in the lung which is somewhat irreversible and begins with not breathing well and progresses to not being able to breath at all.

Black diarrhea implies serious bleeding somewhere in the digestive tract and this can be fatal it is often to mostly caused by aspirin like drugs especially over the counter ones and these are more commonly taken if someone is NOT on narcotics.

Cannot say in your particular case and we are able to give general information only without actually being there to examine, and there are many serious possibilities that are quite likely.
Did I mention that RA and methotrexate make one much more susceptible to dying from infections?

also, I might mention, since I probably have it, that the textbook theory of the physiology of fibromyalgia is a lack of functioning opiate receptors. so giving narcotics to someone with fibromyalgia makes as much sense and me, a male, taking birth control pills for prevention of pregnancy. Both might work if we get sick enough from the pills, I suppose.
