I have gone through your query with diligence and would like you to know that I am here to help. I am really sorry to hear about your daughter, but from my experience I have never heard of any such serious implications (never anything permanent) caused by Depo-Provera. But just to make sure, I did do a little research only from trusted medical sites just to see if there were any such reported cases, and unfortunately there were no reported instances from any part of the world.
The only gastrointestinal implications from Depo-Provera shots were nausea, abdominal bloating, anorexia and abdominal pain. There were no other reported instances other than this.
But if you do not mind me putting forward a few possibilities, please do forgive me if I say something out of turn:
+In most cases with young adults, we can never be sure as to how much drug or alcohol consumption has taken place simply because the young adults do not want a judgemental intervention to take place, and most times also because they wouldn't want their parents or elders to find out. I am not saying that this is the case with your daughter, but it is possible that she has consumed more than you know.
+May be there were some pre-existing conditions that were aggravated by the use of Depo-Provera
+If she was any other medications, may be there was an interaction with some other medication or drug she consumed
