                                            It feels like a sharp sting when I strain during constipated bowel movements. Almost like a pinch or a sharp poke. It started about four days ago but has not gotten worse.

The abdomen needs to be examined. If the pain is located in the right iliac fossa region then rebound tenderness, guarding and rigidity needs to be looked for. The back should also be examined for any tenderness. Appendicitis needs to be looked for. If the pain is in the right upper quadrant pain then it can be related to cholecystitis.
Stool routine examination plus ova and parasite, ultrasound abdomen and pelvis, CBC, ESR and urinalysis should be advised to workup the cause for these symptoms. Treatment can be advised accordingly.
