It is looking like erythrasma.
I treat such cases with ointment clotrimazole and have seen excellent response with that.
Apply ointment clotrimazole for a week.
This should give you response.

I think this is having some effect, but it's definitely not working quickly. What other options would there be? Medicines available only by prescription would be helpful as well. I think my dr might be willing to prescribe without another visit.
