Question: I have swollen and painful glands on the right side of my groin. It came on over the course of 3 days on the second day suddenly I noticed a quarter sized patch of painful blisters.on my right upper thigh. The patch is hard,inflamed...red and is very painful. I have been feeling fatigued and achy. No fever

Hello,

From your description, I think you have shingles.

And don't worry that you didn't have the shingles vaccine, because the currently available one is only 50% effective.

Shingles is caused by a virus similar to genital herpes, but called herpes varicella zoster. It is the chicken pox virus you had as a child come back out as shingles.

You will need a course of Acyclovir. I see that you have some for your genital herpes treatment. The usual treatment for shingles is: 800 mg orally every 4 hour while awake (5 times daily) for 7-10 days.

The SOONER you start the acyclovir the better! If it is started within the first 72 hours of the eruption, it greatly decreases the chances of post-herpetic neuralgia (pain after the shingles resolves).

The achy fatigued feeling is part of the prodrome that people often experience before and as the eruption is coming on.

Here is what I suggest:
1. Take a dose of acyclovir now!
2. Go in to the urgent care clinic to confirm the diagnosis and for the prescription for the larger dosage of acyclovir that you will be needing.
3. They will likely also give you some pain medication that is stronger than the naproxen, such as Tylenol #3 (with codeine).
