Question: I have a swollen left ankle with a bullseye type rash and severe itching. I worked in the yard yesterday and itching started last night! What should I do?

Noted Your Medical History ,which says you are diabetic and has other medical problems. This makes me worry as there are chances of more complication than anticipated in a patient who does not have diabetes. I would suggest you to consult a Doctor , start an antibiotic cover to prevent secondary infection, antihistamine to reduce the allergic reaction and itching. Elevate the leg above the heart level in a lying down position to reduce the edema. Check the blood sugar level today. Wish you an early recovery
