What Causes Swollen Ankles While Treating Stage-4 Non-small Cell Lung Cancer?

Question: My brother has stage 4 NSCLC, and is on a huge cocktail of meds, including 2 fentanyl patches to equal 125 mcg. Dilaudid as needed, plus regular stuff like Ritalin, Allegra, Advil, acetominophen, olanzipine, prozac...etc. he has spent the past 9 months undergoing radiation therapy, chemo with carboplatin and alymta, maintenance chemo, and now has had one dose of obdivo. Rapid changes in meds have occurred over the past three days, and now he has swollen ankles. Cogentin was started today. Does he need immediate medical attention?
