Question: i have swollen nodes in right groin area. fatigue abdominal pain, low back pain on same side. area is red and swollen. started as hard lumps with headache. pains into rectum.

Lymph node enlargement is the most common cause of swellings in the groin region.
Lymph node enlargement is caused by infection, inflammation of the area which drains into the lymph nodes.
Malignancy-primary called lymphoma or metastasis from some other site are other causes of lymph node enlargement anywhere in body.

Painful swelling associated with redness of the overlying skin is more suggestive of an infectious pathology rather than malignant pathology.

Is there history of urinary tract infection (UTI) or infection in the lower limb?
Lower back ache is associated with urinary tract infection, thus if there is history of UTI, lymph node enlargement can be explained.

Whatever may be the cause, an aspiration cytology of the enlarged lymph nodes is warranted.
Aspiration cytology is a simple, safe, office procedure in which cells are aspirated from the swelling with a fine needle.
The cells aspirated are spread on slide, stained with appropriate stains and viewed under microscope.
The origin, nature and character of lesion can be assessed by this investigations.

Fatigue suggests an infectious nature of lymph node enlargement.
Abdominal pain can be related to the above mentioned symptoms or can be due to gastrointestinal problem.
More clinical details are required for the interpretation of abdominal pain.

History of swollen and purple toes suggest some vascular pathology in lower limb.
A consultation with a surgeon is required for clinical assessment and relevant investigations.

no history of infection as uti or swelling in toes. In fact I noticed hardening of nodes and fatigue starting before toes. could they be un associated? thought perhaps spider bite. started small at base of one toe. then spread out to two more. one more than other. did find two deer ticks attached to body about 2 weeks before symptom s began. also wondering if Lyme possibility? will 're contact primary care tomorrow.

A history of spider spider bite could be a cause of lymph node enlargement.
Some spider bites can be poisonous enough to cause lymph node enlargement in the groin area.
Exposure to ticks also raises the possibility of Lyme's diseases especially when it is endemic in your part of word.

I would suggest you to consult your primary health provider for an aspiration cytology and serological test in case Lyme's is suspected clinically.
