Question: Over the last year I have had a growing amount of pain, both tingling and aching in my hands and feet. Some days it's pretty irritating. I have had three levels of fusion on my lower spine, and I've had some issue up higher that have not been diagnosed. I've tried Arrosti, and other treatments but the pain in my hands and feet continues

I can understand your concern. You are having pain and tingling in all extremities. It could be due to

: Compressive lesion at cervical(neck) and lumbar(lower back) region
As you have history of fusion in lower spine and some undiagnosed problem in neck region, compressive lesion at both sites could be your cause.

We can suspect it more if your pain is sharp, shooting, radiating from neck/lower back to extremities, pain increases on flexing back or neck and there is some functional loss (sensory and motor). Consult your doctor if you have such symptoms.

He will examine you, advise X ray of neck and back/MRI of both region to confirm.
After confirmation, you can be treated by
; Avoid weight lifting
; Avoid neck flexion and forward bending of neck
; You can wear neck collar and lumbar corset to prevent flexion. Do not wear more than 8 hours a day.
; You can take Pregabaline/Gabapentine/Amitriptyline or combination therapy after proper prescription
; You can take S-adenosylmethionine after proper prescription.
; Surgery may be needed if no relief in symptoms and there is significant compression of nerves/spinal cord or along with compression, you are having functional loss.

: Another reason could be "Peripheral neuropathy'.
It can be suspected as your pain and tingling are only in extremities not in neck/back, some sensory or power loss in hands/feet, any history of diabetes/chronic drug intake/alcohol intake.

Consult neurologist for it. It can be confirmed by evaluation, blood investigation like B12 level/sugar level/kidney and liver function test, nerve conduction study of peripheral nerves, electromyography of hands and feet muscles and sometimes biopsy nerve.

I have mentioned all important tests to confirm diagnosis and differentiate between two.

Treatment of peripheral neuropathy is medical only. Treatment of underlying cause and symptomatic treatment like painkillers mentioned above.
; Avoid sugar/alcohol/drug abuse of there is any history
; You can also take nerve specific vitamin like B12/folic acid/Pyridoxine/thiamine
; You can also try Acupressure therapy.
; If there is no pain relief even after all drugs, consult your doctor about opioid analgesics like pantazocin/hydrocodone/oxycodone
