Question: I have Type II diabetes. About 6 or 8 months ago I was given a Rx for a blood thinner. Now I have what looks like something tunneling just under my stomach skin where I inject Novalog and Lantus. My weight is gaining and taking Torsemide doesn't seem to help. My breath is labored after walking a short distance. What is causing the "tunnel" effect? Norm Sielisch YYYY@YYYY Thank you

This may be initiation of lipodystrophy. This is a known effect of insulin as it dissolved the fat from the local area where it is applied more than the rest of the body. It is usually a harmless condition but does not look too good. So cosmetic issues are the main concern of most patients once this condition starts.

The best way to avoid this condition is to keep changing the site of injection of insulin. You can use right thigh upper half, right thigh lower half, left thigh upper half, left thigh lower half, right arm upper half, right arm lower half, right upper quarter of abdomen, right lower quarter of abdomen, left upper quarter of abdomen, left lower quarter of abdomen. You can use one site in one day and move to second the next day. The rotation of the sites this way usually helps avoid the condition.

As for breathlessness, I am assuming the blood thinner was started for a cardiac condition. So it would be my educated guess that you will have to settle the cardiac issue first. Low cardiac functions can also cause trouble and similar breathlessness. Of course very high sugars can cause a condition called diabetic ketoacidosis which has breathlessness as one of the symptoms, so check your sugars as soon as possible.
