What Causes Vertigo And Hearing Loss After A Single Loud Sound Exposure?
I am an epidemiologist (MD, PhD). I have been working in XXXXXXX Leone for the last 6 months. On Sept 26, 2016 around 11am I opened my bed room door and heard a very noisy generator. My ears blocked as if I was flying in the plane. I tried to clean my ears but the blockage didn't resolve immediately. After few hours, I started to have vertigo. The vertigo was severe at night and I vomited 3x and in the morning I was admitted to a private hospital. Investigation was done (CBC, liver function test, blood chemistry and ECG) and everything was normal. But I still felt blockage and vertigo when I walked or stood up. My hearing ability in my right ear was impaired. I was discharged and the vertigo reduced a bit. After a week, I went to Addis Ababa and was seen by ENT specialist. He tested my hearing ability and made a diagnosis of right ear deafness due to unknown reason. MRI was done and it revealed nothing(everything normal). I took 10 days steroid treatment and I came back to XXXXXXX Leone. The vertigo is still happening but reduced. I have also a tinnitus (continuous one) since the very beginning. The tinnitus is really disturbing. I went to an ENT clinic again in XXXXXXX Leone and at this time, he observed my eardrum very carefully. He tested my hearing ability and he found that I could hear in both ears but reduced hearing ability in the right ear. He found that the right eardrum is depressed due to congestion in in the Eustachian tube. He ordered me a nasal drop and valsava maneuver. I have been following these medication for sometimes now. But the tinnitus is still persisting and there is also occasional fullness and dizziness in my right ear. What could be the problem?

From your history it is quite clear that vertigo and hearing loss occured after a single loud sound exposure.
This is sudden sensoryneural hearing loss..due to nerve injury by a loud sound exposure
In most of such cases in my clinical experience, it takes around 3- 4 weeks for the vertigo to resolve completely and hearing loss is not cured 100% but it surely improves if the patient is put on steroids at proper time. In such cases, steroids have to be continued for around 3 weeks in tapering doses.
tinnitis will improve as hearing will improve.
Eustachian tube dysfunction if present aggravates the whole situation, for this you need to take a decongestant orally as well as per nasal..also take steam inhalation.
Take vit b12 1500 micrograms once daily.

there is no role of steroids orally now at 2 months..you should visit an ENT specialist..nowdays i am getting good results with intratympanic steroid injections in such cases, so you can consult regarding that
take vit b12 for atleast 40 days
for eustachian tube dysfunction, yes you continue with nasal drops but also take oral decongestant and steam inhalation
do valsalva exercises
