Question: Watery blisters on shoulders last night. Compromised immune system due to MS medications. Headache and ache in shoulder joint. Headache becomes severe on back of my head, same side as blisters. Red spots only now. No fever. I had chicken pox as a child

Possibility of herpes zoster is highly likely in this scenario.

You should visit to a Dermatologist or Physician for examination and evaluation.

You have to be started on anti-viral medication i.e. Acyclovir or Valacyclovir along with local Acyclovir cream and Pregabalin or Gabapentin for pain.
