Intestinal infection. Sequel of gastric bypass.
Detailed Answer:
Hi.
Thanks for your query and an elucidate history of the past of ruptured appendix at the age of 6, removal of left Fallopian tube, gastric bypass and cholecystectomy in 2010. Also noted your medication of Sertralin, robaxin and klonopin.
The present problem is of yellow stool for 3 weeks, mostly diarrhea, pain in flanks - left more than the right - pain and tenderness in abdomen- hypoglycemic (43) episodes blood insulin at 42.3 (normal is around 8) Negative for Celiac Chrohn's, normal colonoscopy yesterday, Wants to know the cause and next step.
I hope the stool has been tested.
The possible causes can be :
Intestinal infection or disturbed GI bacterial flora, causing sort of entero-colitis with attendant pain and tenderness.
The gastric bypass is a string metabolic surgery and cause hyperinsulinemia and low blood sugar levels, the effect enhanced by intestinal infection.
I would advise the following, since some tests have already been done:
Test for stool for infection.
A course of an antibiotic, metronidazole, probiotic for 5 days; to see the results.
To monitor blood sugar levels as this is practicable.
CT scan of the abdomen particularly for Pancreas from where the insulin is produced.
Further course of action after 5 days of treatment.
To take the help of a Gastroenterologist and an Endocrinologist to get a detailed insight of the problem and a probable solution.
I hope this answer helps you to get a relief, an early diagnosis and proper treatment.
