I have completely reviewed your health query, and can well realize all those concerns. Being your physician, I will take care all of them today and will give extend my expertise to solve the problems.
It is really a problematic situation for your grandfather and I want to assure that it is manageable. So don't worry.
It is due to inflammation and irritation of the eyes and with proper medication he will get better. There is a slight yellowish tinge in the eyes and it raises a suspicion for jaundice. You need to test him for that. Go to the lab and so the following tests:
CBC with ESR and Differentials.
Liver function tests.
HBsAg and Anti HCV by ELISA.
Kidney function tests.
If I was his treating physician I must have prescribed him Femicone eye drops and Systane Lubricant eye drops. Plus Cetirizine tablet once daily by mouth. Also get him checked up up by an Ophthalmologist if you feel nothing is working anymore for him.

It can be combined with pale and yellow skin, Yellow to orange color of the urine and severe itching and pruritus on the skin. Stool color may also change. Can be associated with Fever and pain on the right side of abdomen.

Yes diabetes mellitus and hypertension may cause eye issues. As aforementioned, you should take your grandfather to an Ophthalmologist at least once. He will examine the eye and will see the inner structures of the eyes with an Ophthalmoscope. If there are vision problems, it causes lot of eye stress and this may also cause redness and watering. In such a case Ophthalmologist may suggest some eyeglasses for your grandfather.
Less likely but bacterial infection also may cause redness but I don't think he has this infection.
