What Could Cause Recurrent Abdominal Pain Extending To Genitalia And Groin Area?

History of on and off pain abdominal pain for over 2years radiating to genitalia and around the groin region with no other significant symptoms as you mentioned rules out the pancreatic , appendix, liver, bladder causes.
Meanwhile the above features indicates the possibilities of small uretric stones,as they are known to cause the unbearable, cramping kind of pain and it lasts for more than 10 minutes.Pain disappears as the stone move out of the through the urine(size of 0.5mm) and the frequency of the pain depends on the stone obstruction of the ureters.
You had mentioned about the worsening of the symptoms with Vicodin, the ureteric stone symptoms are known to worsen with Vicodin because it causes ureteral spasms.
Only after clinical examination and tests like an abdomen scan or abdomen X-ray. can help in arriving at the proper diagnosis.

1) If uretric stones were the cause to this chronic sporadic pain, wouldn't I feel pain as I pass the stone through the urethra?
Only the big stones can cause pain while urinating(that is while passing through urethra), here we are discussing about the possibilities of small stones(0.05mm),the small stones appears big enough when they travel through ureter because ureters have constrictions normally and even a small stones get stuck in the ureter but not in urethra ,the diameter of the urethra is bigger than the ureter,so small stones passes out easily.
2)Where do these stones originate in the body? Are these the same as kidney stones (excessive calcium)?
Stones originate from the food we consume and the XXXXXXX complex metabolism that happens in body.
Ureteric stones is usually originates from the Kidney .
Not all stones have to be due to high calcium, there are other causes like phosphate stones , urate stones ,etc but all these presents with the same features.
3) I have no lower back pain asociated with these symptoms,if so. Some of the onset episodes, I did not urinate either before the onset of pain, nor after. The pain simply goes away.
Not necessary that all stones have to present with lower back pain, pain origin and progression depends on the site of the stone and the size of the stones.
Not necessarily that pain as to aggravate following urination nor the urination has to relieve the pain.
As these small stones passes through the ureter(only during this period there will be pain)as they reach Bladder and further down ,nothing will be there to obstruct them so they remain silent and goes off in the urine without any symptoms.
4) I have had constipation associated with the onset of the pain
As you mentioned in the history,the initial episodes were not associated with the constipation only the recent few are associated with the worsening of the pain ,though the exact reason cant be drawn in this situation it might be co-incidence or the loaded bowel as result of constipation might have put external pressure over the ureter as these structures lies in close proximity.
