What Do High T Cell And CD8+ Cells Count Indicate?
Question: My absolue lymphocyte count and CD3+ absolute T LYMPHOCYTES ARE HIGH,ABSOLUTE COUNT CD8+T HELPER CELLS ARE HIGH
Brief Answer:
Hi Can you please send me your complete blood report and problem details.
Detailed Answer:
hello sir,
I did review your concern.
High T cells and high CD8+ T cells can be elevated in various conditions including viral infections, some tumors, recovery from bacterial infections and so on.
I need to have a look at your complete blood report sir and also a brief history about your problem.
By that I can give a more complete answer and also advice you on the steps you can take after that.
wish you all the best
Awaiting your reply

Hi I did review your reports and following are my recommendations
Detailed Answer:
Hello I did review your query and your reports sir.
Your blood count is essentially normal except for high CD3+ cells. However, The ratio of CD4 to CD8 is normal. This is essentially a very good sign as it indicates adequate proportions of both types of cells. The CD3+ cells can be high due to various reasons like recent infections, surgeries or recovery from trauma and so on.
I believe the doctor from PGI XXXXXXX got this done to rule out a T cell lymphoma many of which can present as a nasal mass.
Did you have a nasal mass or anything was noted by doctor on the operation?
In any case the chances are very unlikely for any dangerous sign, but to be cautious enough I would recommend-
1) Get a peripheral blood smear examined by a hematopathologist.
2) Consult your doctor if you notice any swelling in any part of the body or any nasal mass.
Since, your blood count including RBC, WBC and platelets are essentially normal there is nothing much to worry about. But I have given following recommendations just to be cautious and rule out any problem.
I hope this helps
wish you all the best
Let me know if you have any more questions or concerns.
Sincerely,
Dr Parth Desai.

Thanks, Yes I do have polyps, which were removed during extensive surgery in Calgary,.attaching pre surgery CT scans-ordinary and carosal, and also CT done at after 4 months of surgery. You will see that in pre CT scans shows extensive fungal involvement. Dr XXXXXXX PGI also mention some present signs of sinus.
in CBC done in XXXXXXX showing eosinophils high .
Does polyps are like /similar to mass stuff.
my IgM was low, ESR high,

Brief Answer:
hi polyps are benign masses mostly but biopsy is definitive.
Detailed Answer:
Hello,
thank you fot your reply query.
nasal polyps are mostly allergic in nature and your high eosinophils suggest that. A biopsy however after surical removal is prudent to see the exact nature of polyp.if you still have polyps then I would advice removal and histopathology too confirm their benign nature. You ct scan reports initially showed fungal infection which also can be confirmed by nasal scrapings biopsy and culture.polyps can occur with fungal infection too and look very similiar to nasal allergic polyps and both have high eosinophils. But fungal involvement needs antifungal therapy as in ketoconazole or amphotericin in severe cases. Also please rule out diabetes as a cause of nasal fungal infection. Your cd 3 counts can be high due to previous fungal infection.it also indicates your cell mediated immuntiy is good enough to battle them off.
let me know if you have any more questions or concerns.

That was the major failure on part of ENT surgeon who has done surgery- not done histopathology, yes I am Diabetic - taking total 4 metformin 500 mg -two morning two evening, Being severe obs sleep apnea - worried for repeat general aneastheia surgery- what do you think- that polyps may be a serious concern of begin in this case- because lymphocyte count -CD panel are high, also linked with low IgM, suggest tests--
Brief Answer:
Hi there is nothing much to worry right now.
Detailed Answer:
Hi, thank you for your query.
I do not believe that there is anything serious as cd4 to cd 8 ratio is normal.
I would recommend-
1)peripheral blood examination by a hematopathologist to look for any suspicious cell, if any.
2)a nasal examination to see for any residual polyps and also lymphnode assessment by a physician.
however these tests are just to rule out any serious condition.
I largely believe those were allergic polyps with fungal superinfection and removed.
I hope this helps
wish you all the best.

As you said- were allergic polyps with fungal superinfection and removed.
does these can be reappeared after surgery?
does these can be treated with medicine?
so these should be tested after surgery -is this correct?
in case treatment delayed than does such infection can effect brain and eyes?
What does PGI HOD note says-- ? what does he noticed in post Scan-- and what does he noticed in his endoscopy?

Brief Answer:
allergic polyps can reoccur
Detailed Answer:
Hello sir,
thank you for your reply query.
the pgi XXXXXXX Hod notes have mentioned that there is haziness in maxillary sinus, however the ct scan shows normal scan reports with normal sinus mucosa and slight hypertrophy of turbinates which can happen with polyps.
Basically allergic polyps can reoccur.but that is not worrisome. The worrisome part is superimposed fungal infection.
For polyps there is no medical treatment other than keeping yourself free from nose infections and allergic substances. For the fungal infection, you need to monitor your blood glucose levels tightly and if there is slightest pain or redness in nose consult an ENT doctor and get yourself treated.
Untreated nasal fungal infections can affect eyes and brain but if treated properly and early it can go away easily.
Right now since your ct scan shows normal findings, nothing much to worry about.but if you have any stuffiness of nose or pain in nose or discharge or any swelling noted please consult your nearby ENT doctor and get examined using speculum and take antifungal treatment if needed.
Apart from that if possible just get a peripheral smear examined by a hematopathologist.
Rest there is nothing to worry about sir right now. Eat healthy food, maintain good sugar control and keep exercising regularly.that should keep you fit and fine.
I hope this helps
wish you all the best
Let me know if you have any more questions or concerns sir.
Sincerely,

Few blood work got back from PGI attached--
Brief Answer:
Hi Selective low IgM is seen on reports.
Detailed Answer:
Hello sir,
I did review the latest PGI reports.
These reports do show selective low IgM levels.
These can be seen as primary immunodeficiency problem or as secondary to allergic conditions, gastrointestinal disorders and so on.
Low IgM does impair immune response to certain pathogens like bacteria to some extent and may be a cause of your fungal sinusitis with polyps.
However, I am not an expert in that field and I would recommend you to consult an immunologist for further managment in terms of prophylactic antibiotics or certain vaccines that should be given or further investigations for subtyping of B cells to be done or not. Your T cells appear normal and so that is good news as far as your T cell immunity is considered.
I hope this helps.
wish you all the best.
Let me know if you have any more concerns or questions.

Brief Answer:
Here are answers to your questions sir
Detailed Answer:
Dear Sir,
thank you for your reply query.
I think Dr.sehgal in PGi is a very good immunopathologist and that department itself can guide you further tests. The tests mostly would include some antigen sensitivity testing, B cell tests including flow cytometric analysis of your blood B cells.
Vaccines that would be greatly useful in your case would be Pneumococcoal polysachharide vaccine and meningoccocal vaccines. Prophylactic antibiotics would be something on lines of azithromycin 500 mg once a week or ampicillin course, however proper antibiotic coverage can be initiated only by your doctor or immunologist since one has to take into consideration your past infections and rate of infections. vaccines can be taken irrespectively.
Selective IgM deficiency can be seen rarely in malignancies but Your lymphocyte count is normal so atleast a CLL is not on mind. But as I said a peripheral smear examination by a hematopathologist will help in ruling it out.
It is rarely associated with gastrointestinal malignancies but you should get work up for that done only if you are experiencing weight loss or abdominal pain or any changes in your bowel habits.
I hope this helps
Wish you all the best sir
