What Do My BP Readings Indicate?

Question: My bp is 113/61, should I take my Carvedilol 12.5 mg at bedtime. I took ativan 1 mg at 5:30 when my bp was 136/74, now at 8:15 it 113/61, pulse 77. Should I hold off on the carvedilo?

Hello , I am Dr Mody and I would be addressing your concern.
The indication for carvedilol is not just for control of blood pressure, it keeps your pulse stable and is for protection of your heart in most situation rather than it being used for control of your blood pressure , so skipping dose is not wise .
And ativan is not antihypertensive and it would have decrease the anxiety which was keeping your pressure up .
As for now , take half a tab carvedilol (6.25) effective dose ,and go to sleep , it will not decrease your pressure much .
Also if your blood pressure stays in this range you may consider decreasing the dose after consulting you physician .

I take Carvedilol 12.5 mg, 1 bid and also Lotrel 5-20 mg., 1 daily. If my blood pressure contines to run low what medication should be lowered. I recently had aortic valve replaced with a cow valve.

Hello , sorry for being late as I was tied up .
Now the medication you are taking will reduce the blood pressure .
Though if the blood pressure is maintained at more than 110 / 70 , it's ok , that's not considered low blood pressure.
Carvedilol and lotrel to a lesser extent has many effects on the heart rather than just decreasing the blood pressure.

It will help in increasing the pumping capability of the heart which would have just started decreasing because of previously stenotic valve .

It will protect against various arrythmia -imbalance in rhythm of the heart

It will decrease the load on aortic root (the biggest artery of the heart - helping it's healing )

These are just some important pleiotropic effects on the heart .
So optimisation of the dose to maximum possible dose is recommended after early replacement of the valve.
However if the pressure are on lower side , it's better to decrease lotrel rather than carvedilol as that is a more powerful antihypertensive and is the reason of the lower pressure to have .

I had a-fib after open heart for valve replacement. Was given Amiodarone at hospital ans was on it for 9 1/2 days and was taken off because of my liver tests. What is the half life of amiodarone. It also induce hyperactive thyroid. Now, they are removing my thyroid. What is the possibility of have a-fib again?

Yes amiodarone is a long acting drug and it's action will continue even after its stopped , also it can cause hyperthyroidism transiently .
The half life of amiodarone is 58 days and it's active metabolite will work for 35 days after it was consumed last .
However atrial fibrillation may be the result of hyperthyroidism state and so it should be addressed .
The hyperthyroidism because of amiodarone would revert back to normal after stopping of medication .
As they are removing a part of your thyroid gland rather there will be euthyroid status or hypothyroid status , in both there will not be atrial fibrillation as it is because of excess thyroid hormone and not normal or less than normal hormone .
It's good we didn't decrease carvedilol as it's one very important medication to prevent atrial fibrillation.
