I have gone through your query. Your fasting as well as post prandial blood sugar is high. This can be managed by oral hypoglycaemic drugs.
In this case if you were my patient i would suggest to start metformin therapy. Lipid profile should be checked if high statins like Atorvastatin also will be given. Continue Telma for hypertension.
Regular exercises like brisk walking or jogging is good. Diet is also very important. Increase more fibre rich food and food with low glycaemic index.
If there is no improvement with Metformin , diet and exercises then i would suggest adding DPP4 inhibitors like sitagliptin.
