Please review my reports, I also had an accident last year and since then having a back ache issue, unable to sit more than 1 hr and also can't sit crossed leg more than 10 mins. When i lie down at that time, having a pain which sometime goes to left thigh. I can walk for 2 hours without any issue with some strain on back. I am still trying to figure out how this issue can be resolved which is stopping me to do activities as mentioned above. I had several physio therapy sessions but still issue remains. Please advise.

I am Dr Sudhir Kumar, Neurologist, and I would try my best to help you.
I have noted your clinical details and also seen your mRI reports. Based on this, there is a compression of nerve in lower back, mainly at L4-5 level. This is most probably related to the accident.
Initial treatment includes medications (such as gabapentin or pregabalin) along with physiotherapy. Since this treatment has not helped (which happens in 30% of patients), you can opt for epidural injection.
In this procedure, an injection is given in the lower back (at the site of compressed nerve). This results in pain relief for long periods. Physiotherapy exercises can be continued.
Avoid lifting heavy weights and forward bending.

I agree with your plan. Physiotherapy does take time and if your symptoms are not so troublesome, you can wait for good relief to come slowly.
I agree that epidural injection involves steroid injection, however, it is safe as a small dose of steroid is given on only one occasion. Steroids cause side effects only if steroid tablets or injections are taken over long term (months to years).

thanks sir for your response. I consulted lot of doctors in between and all said after physical checking my conditions that this not related to the accident but yes one of the neuro physician also said like that this might be related to the accident. Epidural injections is kind of steroids and I really don't want to take these. I am currently started going to QI spine clinic and they mentioned that my conditions can be improved after long sessions for 2.5 months or so. I may take some time to review but may think of any other alternatives if QI guys will not able to help me.
