Question: I have a 1.5" sore on the left side of my testicals. The skin is hard and very tender. This started about a week ago. I am married and have been exclusively with her for 14years.

Noted the medical history, history and understood your concerns.
The sore on left side of scrotum may most probably a skin infection as you have diabetes too.
Please give additional information for me to assist you better:
Please post clear and focused photos.
Is diabetes under control - get blood sugars and HbA1c and post reports.
I hope the lesion can be moved over testis freely and easily means testicle and epididymis is free of skin lesion- please check and give me the feedback.
Any other information that you would like to share.

If there is redness, more warmth get started an antibiotic also and take Ibuprofen thrice daily. Apply Liquid povidone iodine 2- 4 times daily.
