What Do The Following X-ray Scan And Bone Density Reports Indicate?

Question: ok here is exactly what I need to know specifically.

I already had a d e x a scan and my bone density is good both positive in t and z. I had this after my hip fracture to find out what may have caused it. The results were good no osteoporosis or any sign in bones weakening.

My fracture was five weeks old before I had an MRi.

Why would I develop sclerotic bone formation? instead of calcium or ossification?

wouldn't the mri scan picked up sclerotic bone formation?

wouldn't the x rays pick up sclerotic bone formation?

wouldn't a c arm pick up sclerotic bone formation?

can sclerotic bone formation occur in five weeks in a new fracture that has not yet been diagnosed as a hip fracture?

why would i have sclerotic bone formation in a new fracture?

what does the surgeon do with a patient that has sclerotic bone formation that has develop to perform surgery and install a long gamma nail and dynamic hip screw?

does he have to remove it?

if so how is this done?

the long gamma nail was implanted to stabilize the fracture. but im not sure why because it was already stable and non displaced.

after the surgery I had non union that went on for months and then delayed non union.

I kept falling because my leg was still weak I had poor balance and coordination with extreme pain still radiating from my right buttock down to my knee.

the doctor would just keep telling me that the fracture has not healed yet. This was seven months after the surgery.

I had another c t scan a year after the fracture occurred and ten months after surgery.

the c t scan showed the fracture was not completely healed.

it also showed a butterfly commonunited fragment and a fragment in
the soft tissue that was stable.

it showed severe heterotopic ossification in some areas.

and it also showed sclerotic bone formation?

I had another c t scan performed 8 months later and it is still not completely healed?

I still have poor balance and coordination with extreme pain and weakness. I can only stand or walk for ten minutes then I have to sit down because the pain is so severe.

I'm wondering why my fracture has not yet healed almost a year and a half later.

What are your thoughts?

im thinking the implants didn't work or the hardware failed and the fracture tried to heal itself. but I don't know why I had butterfly communicated fragments and a fragment in my soft tissue that was not there before?

it seems to me this is a different kind of fracture than you previously had?

i did not have any fragments out of place with my fracture. the fracture was non displaced stable complete in good position and alignment nothing had moved everything was still in place. it was a very small fracture similar to a stress fracture.

how long should the surgery have taken?

Hello,
I have studied your case.Non union in bone can be caused bu both local and systemic factors. Local factors like comminuted fracture, infection, in proper implant can cause non union. Systemic factors like low vitamin D3, Low calcium can also cause such issues. So I think in your case both causes can be there. I would suggest you to please send me your current x rays. Also let me know if you have any other comorbidity like Diabetes, Hypertension or osteoporosis?

Ok I do not have a history of any bone abnormalities osteoporosis high blood pressure high cholesterol or diabetic. I had a d e x a scan performed on 7/2014 and I have good minerals supply for a person of my age both positive in t and z and no signs of osteoporosis etc.

I developed high blood pressure in 2012 from a medicine called effexor. I now take norvac.

I was just diagnosed in March of 2015 with high cholesterol and I was pre diabetic at 6. 2. I also was diagnosed with vitamin d deficiency.

I started taking l ipitor for my cholesterol vitamin d 50,000. for my pre diabetic condition I have changed my types of food And I have lost 53 pounds from XXXXXXX through August. My pre diabetic level is now 6.0. I take a vitamin for women over the age of forty that includes vitamin d vitamin b12 vitamin c iron potassium vitamin b12 calcium vitamin k etc. I have not started menopause.

I am still unable to climb stairs by myself tie my shoes lift ny right leg to dress without holding onto my dresser. I have to sit on the side of the tub to shave my legs. I can no longer take baths because I do not have enough strength in my right leg to get up. I cannot kneel down I can barely bend over I can only walk of stand for more than 10 minutes or severe pain starts and I need to sit down immediately or I fall. I cannot climb on a chair get up from deep sitting positions without help no physical activities or exercise I have to use a XXXXXXX shopping cart. I have to be dropped off in front of places we are going. I still have severe weakness in my right leg and poor coordination and poor balance.

I was also taking tramadol during the time of the healing of the hip fracture. When I was researching all the side effects of my medicine and drug interactions I found out that tramadol and effexor have a major drug interaction called serotonin syndrome and this can cause poor coordination muscle spasms and confusion. I discovered this in 7/2015.

I only have side effects from medicine no allergies.

I gained almost 60 pounds from 2014 through XXXXXXX of 2015 as a result of my hip fracture pain and limitations. This caused me to become pre diabetic and have high cholesterol. My vitamin d is low because I no longer go out in the sun like I used too. I was an XXXXXXX swimmer and love being at the swimming pool. This all stopped as of 10/29/2013 and continues til this day. I have lost 53 pounds since XXXXXXX but my blood sugar levels has only dropped to 6.0. I have changed all the type of foods I eat.

I do not have any images I can upload they are located in a disk I have. I do however have all my medical records from 11/2/2013 through 6/16/2015. These include all the radiologists results from all the x rays c t scans c arm images results bone density scan and surgical post operative report consultation operative report and pre operative reports. So let me know if you want any of this information.

I also have the mri report from the radiologist from 12/4/2013. This was preformed under an open high field tellsa 1.6 mri machine. This may need to also go to the radiologist who is answering questions too.

I have sent reports today.

I did not have a communicated fracture see mri report from 12/4/2013. I had an intertrochantronic right hip fracture that was non displaced with edema. it was stable oblique pattern very small similar to a stress fracture. it was a complete break but still in alignment nothing had moved out of place and in good position. that's my question? the mri was done on 12/4/2013 in a open high field Hitachi 1.0 machine. it did not show any type of communities fracture or fragments. it did not show any signs of healing and it did not show any sclerotic bone formation. I do not have any diseases disorders osteoporosis multiple sclerosis problems or pageants disease no cancer nothing at all. no vitamin deficiency bone density is healthy above both normal ranges in both the t score and z score. the only mental health issues I had were mood disorders and high blood pressure. I weighed 236 pounds@the time of the surgery but the hospital weighed me wrong. I saw my primary care doctor on 11/26/2013 @ 11/8/2013 and I weighed 236 pounds. im trying to upload the actual mri x rays c t scans to you but I haven't heard back from customer support on how to zip these images from my scan disks and upload them. as you may know and mri takes many images and so . do c t scans. maybe you can help with contacting someone to call me. 513 330 3402.

Hello,
I have studied your reports and I think you are having delayed union due to weak bones. It may take two to three more months for complete union. I would suggest you to take bone stimulating therapy for increasing rate of union.
apart from this following are options.
1.Bone marrow protein( BMP) injection can be given under image.
2.Some time Bone grafting can be done if above mentioned methods not working.
3.Take calcium and vitamin D 3 daily.

I developed vitamin d deficiency because I don't go out side anymore because of my hip fracture. I do not get sun exposure. I was diagnosed in March of 2015 when I had a complete health physical. I now take vitamin d 50000 once a week and my levels are normal. I also gained almost 50 pounds from my hip limitations chronic pain any really none of my hip fracture symptoms went away. Now I have lower back pain and because I have been favoring my right leg so much I caused my left knee to give out from over using it and I now need knee reconstruction surgery. From the inability to do most any physical activities I am mostly confined to home. Because of my hip limitations and inability to do most physical activities or exercise I developed high cholesterol and im now pre diabetic. I have been under so much emotional physical and mental distress from my hip limitations chronic pain and because I confined to home it has worsened my mood disorders severely. I just can't understand why a small fracture like mine not caused my any trauma or falls would not heal quickly. The longest it should have taken is three months maximum. We are coming close to the two year anniversary. Especially considering that I had no deficiency at all and no diseases no cancer no bony lesions no abnormal bone disorders no osteoporosis no multiple sclerosis really no problems at all. My calcium levels are excellent my bone density is healthy above both positive in t and z. No family history of fractures or bone disorders and I have eight siblings to compare to twenty two nieces and nephews and 27 great nieces and nephews. Doesn't this seem odd to you?

I understand your problem and it is really frustrating to not able to go outside. I would suggest you to discuss with your doctor about options I have mentioned and see if he can plan a bone graft. it is good that your blood vitamin D levels are normal .
I would also suggest you to discuss with your doctor if you can walk with walker support because I think there is plenty of time has passed. Walking will further improve your bone.
