
What Do The Following Blood Test Results When Diagnosed With TB Indicate?

My weight was earlier about 86 kg (before TB was diagnosed). which has now been reduced to 69 kg only.
Serum creatinine was 7.4 on 25th Oct 2015, it has (by itself, without any medication) gradually come down to 1.55 on 18th May 2016. But it again shows mild elevation at 2.2 in report of 29th May from a different lab.
My main concern is loss of protein in Urine and frequent urination (once every 2-3 hour). My blood glucose report was normal
Please advise how should I go ahead , should I simply wait for some time or consult any specialist ?? As I told you my main concern is loss of protein in urine and frequent urination, the wait is not increasing at all.
Kidney , Pancreas ultrasound is normal.

Hi Mr XXXX,
Renal Failure can be reversible if there is a reversible factor, in your case i feel there could have been renal involvement with tuberculosis which responded to TB treatment. Also some people develop acute interstitial nephritis which improves.
But problem is even if it improves it leaves behind damage to the kidneys ( scars). these manifest as proteins in the urine and raised serum creatinine levels . there is a further potential for them to damage the kidney further. So this could be one of the causes of urine protein leak and swelling in the legs.
also sometimes patient can develop nephrotic syndrome ( high amount of protein in urine > 3 gm/day).
So what you can do is 24hrs urine protein and creatinine ratio to see the exact amount of urine protein . also you can see the serum protein and albumin levels.
Frequent urination can be part of use of lasix / dytor medicines which reduce swelling ( if you are taking those) secondly in patients with swelling once you lie down at night, more urine is produced as the swelling fluid moves to the vascular compartment and you need to pass urine again and again at night.

There is no swelling in legs whatsoever. No medication for that also is being taken.
I also dont have any direct medical issue . Appetite is ok. Energy lever is more than better. I play basket ball (10-15 minutes), tt (60-90 minutes) daily. Only thing is "fear of protein loss" and "weight not increasing" .
A family doctor (endocrinologist) has prescribed renal biopsy. but i am very afraid of Renal Biopsy. Some doctors are asking to wait for another 2-3 months so that damage done by TB can be subsided.
Is it possible that all it may be remaining effect of TB and the problem subsides by itself, if i wait for another month for renal biopsy. because almost all parameters are continuously improving.
Also, if the protein loss can be prevented or marginalized by any allopathic medicines ? Please also inform if I can take protein supplements like Protinex etc. ??

Hi,
please get a fresh 24 hrs urine and a fresh ultrasound KUB report.
no doctor should do a biopsy before seeing these reports in your case
you are correct it might be residual damage of TB, but as i told you if protein in high > 3.0 gm per day and ultrasound show good kidney size it may be worth doing a biopsy. Biopsy is a very minor procedure, there is no need to fear it even if doctor prescribes it.
protein supplements should not be taken . 0.8gm/Kg body wt /day should be your protein intake.
