What Do The Following Test Reports Indicate?

Question: Hi Doctor,
my mother 66 years of age is suffering from diabetes. she takes amaryl 1 twice daily before breakfast and dinner . also lipvas post dinner.

2 months back she has taken glycomet 200 post breakfast and dinner for a month then she left as sugar was slain control . recently I have got her diabetes test done. report is enclosed. please advice way ahead.

Need better blood sugar control.

Detailed Answer:
Hi,

Thanks for your question.
Noted your concern.

After going through your mother's test results
- HbA1c of 9 and Fasting blood sugar more than 200 denotes poor blood sugar control.
I may suggest use of long acting insulin (like insulin Glargine) with oral antidiabetic (may start Metaformin starting with small dose with gradual increase in dose to 1500mg with DPP4 inhibitors like Sitagliptin may continue Tab Amaryl)
With subsequent follow up insulin may be stopped with maximizing use of oral medicines.
Better to consult her treating diabetologist as detailed physical examination, face to face counseling & diabetic education is important in deciding use of different types of oral medicines.

- Urine examination is acceptable.

- Lipid Profile is also within acceptable limit, I may suggest to continue Tab Lipivas.

- Renal parameters are also within acceptable limits.
