MRI findings suggest sequel of chronic inflammation in distal anterior supraspinatus tendon.There is a small calcification in distal anterior supraspinatus tendon.
Calcification develops due to long standing inflammation.
There is also small fluid in shoulder joint.It may be due to long standing inflammation in supraspinatus tendon.
Other findings are normal.There is no ligament or muscle tear around shoulder joint.
Reconversion of red bone marrow is a reverse process of natural replacement of red marrow by yellow marrow.It is a physiological response to increased hematopoietic needs of the body and it is seen in many conditions like smoking,obesity,respiratory problem etc.
You are rightly advised for steroid injections.It will reduce tendon inflammation and will relieve your symptoms.

If physiotherapy and steroid shots are not helpful then other options are ultrasound guided barbotage or surgery.
During ultrasound guided barbotage,calcified deposit is injected with a salt water solution and then calcium is also sucked out into a syringe.Involved area is then repeatedly washed.
Surgical excision is the last option.
