What Do These SGOT And SGPT Levels Indicate?
Question: My nephew is 4yrs old. His sgot is 388.4 and sgpt is 727.6 u/l. His blood test shows negative on Anti HAV Igm, HBs Ag( Australia antigen). Anti HB c Igm and Anti HEV Igm. What could be the problem?
Brief Answer:
Could be autoimmune hepatitis or EBV infection.
Detailed Answer:
Thanks for asking on HealthcareMagic.
I have gone through your query and indeed her liver enzymes are raised. The causes related to infective hepatitis have been ruled out. However, autoimmune hepatitis (liver inflammation caused by an autoimmune disorder) is a condition to consider. I would want to know the bilirubin level for confirmation. Other causes could be Epstein Barr virus infection (sore throat), Cytomegalovirus (CMV) infection (may resemble common cold). Clinical correlation is needed in order to confirm.

Thank you for sharing your observation. The bilirubin level is also normal as the report (attached for reference). Direct: 0.18mg/dl & indirect: 0.29mg/dl
Brief Answer:
Number of causes possible
Detailed Answer:
Thanks for clarifying.
Common causes of elevated liver enzymes include:
Certain prescription medications, including statin drugs used to control cholesterol
Drinking alcohol
Heart failure
Hepatitis A
Hepatitis B
Hepatitis C
Nonalcoholic fatty liver disease
Obesity
Over-the-counter pain medications, particularly acetaminophen (Tylenol, others)
Other causes of elevated liver enzymes may include:
Alcoholic hepatitis (severe liver inflammation caused by excessive alcohol consumption)
Autoimmune hepatitis (liver inflammation caused by an autoimmune disorder)
Celiac disease (small intestine damage caused by gluten)
Cirrhosis (early stages of liver scarring)
Cytomegalovirus (CMV) infection
Dermatomyositis (inflammatory disease that causes muscle weakness and skin rash)
Epstein-Barr virus
Gallbladder inflammation (cholecystitis)
Heart attack
Hemochromatosis (too much iron stored in your body)
Hypothyroidism
Liver cancer
Mononucleosis
Pancreatitis (pancreas inflammation)
Polymyositis (inflammatory disease that causes muscle weakness)
Toxic hepatitis (liver inflammation caused by drugs or toxins)
Wilson's disease (too much copper stored in your body)
With the bilirubin levels low, the hepatitis and liver failure related causes get ruled out. But for exclusion of the other possibilities clinical correlation is needed and careful consideration of the history along with proper clinical examination would be essential. I would request you to take him to a doctor for the same.
