What Do These Following Blood Reports Indicate?

Question: Hello Doctor , I am 35 yrs female patient with weight 62 & height 5.3 inches & my recent blood test results are as follws : Sugar (F) is 111,,Billirubin shows normal but Sgpt is 45 & Sgot is 144 ,,,A/G ratio is 4 ,,Alkaline Phosphatase is 114 . Ldl is 114. My Ft4 shows normal but TSH is 12.5 . Hb is 11.6 . I would be obliged if i get some opinion & also want to know whether i can take immediate medicines like Statin , Eltroxin , Sylibon etc as the doctor whom i want to see is out of station & will be back after a month. Thank you.

Hi Mam.

I understand your concern.

You possible seem to have mixed features of Hypothyroidism and obstructive liver disease.

For elevated TSH I would start you on Eltroxin 25 mcg initially on empty stomach daily for 30 days and then follow up with your physician when he returns home. Eltroxin is a prescription only medicine.

Statins and Sylibon are currently not advisable. It would be beneficial to get an Ultrasound Abdomen to rule out gall stones. Further management is based on test results.

Thank you for your quick response. Actually my gall bladder ( for gall stone ) has already been operated 15 yrs back. i had my last ultrasound ( for follow up ) done in March 2014 which has confirmed grade 1 fatty changes in my liver & mildly prominent ( 6.5 mm) Bile Duct ( no SOL & calculi in lumen ) . I am also suffering from endometriosis as is common due to delayed pregnancy & now under Homeopathy treatment .I have got response with that & got relief from cramps so still continuing. They treated my thyroid & fatty liver problem too but that didn't repond much . So as my liver enzymes , tsh & ldl are high now which i think is not good so i am thinking to go for allopathy again . I will see a hepatobiliary specialist but after a month . so for immediate action i need to know whether any medicine is required or not for time being or can wait for a month without any medication for LDL & liver ( as you have already suggested Eltroxin 25 TSH ) . Thank you.

Hi Mam,

I understand your concern.

I advise you to have a fresh ultrasound of the abdomen,since gall stones are known for recurrence especially with your current pattern of symptoms.

As far as elevated liver enzymes are concerned I advise you to go for Tab Livolin forte one cap thrice a day for 1 month.

This drug is known to protect liver enzymes.

For elevated LDL ,you can currently have oatmeal twice a day for 1 month before proceeding to you use medications.

Only after 1 month,you can go for tab Aztor (Discuss with your treating physician for appropriate dosage). This drug will bring down elevated LDL.

Ok. Thanks . I will follow your advice. Will revert to you again if any futher advice needed.
Also just wanted to let you know that at the time of my gall stone surgery my surgeon has also removed my gall bladder along with the stones & it's also mentioned in my last USG report that "gall bladder operated" . So hope stone will not recurr (not sure just my asumption) . But i will still go for a fresh USG as adviced by you before visiting my Specialist Doctor. Thank you.

Sometimes stones can recur in the CBD/common bile duct that is left after surgery.

Hence Ultrasound is advised.
