What Do These Following Blood Reports Indicate?

Question: Hi,

Since last 2-3 weeks my son (3 year old) is suffering from cold, cough, sneezing, and fever. He gets recovered in between for 1-2 days but again gets sick. Last week, I checked with doctor and he suggested below medicine.
SSP Meftal - P
Laveta M
Brozeet - LS
Today, I visited again to doctor. He advised to get the blood test done to check for any bacterial/viral infections, malaria or typhoid.

I got the reports few hours ago. Please find the attached report.
Please advise.

Thanks for the query

I completely understand your concern.
I have gone through the reports and apart from mild neutrophilia(Polymorphs-69%) this is almost a normal report.

The most common cause of cough/cold and fever in this age is some viral infection or allergy.
But the viral infection usually gets subsided within 5-7 days and fever is not common in allergy.
This may be the reason your doctor has advised you to go for investigation of hemogram,malaria and typhoid.

Meftal P contains mefenemic acid(an antipyratic).Laveta M contains montelukast with cetrizine an antiallergic and Brozzet LS contains Ambroxol(mucolytic) with salbutamol(bronchodilator to relieve airway spasm).
All these are conservative treatment to relieve the symptoms.

Here I have few queries-
What is the pattern and severity of fever?
Is there any associated breathing difficulty,headache?
Is there any family history of allergy/asthma?
How severe is the cough(if causing sleep disturbances/vomiting)?

The raised polymorphs can be due to bacterial infection.
One should rule out tonsillitis(inflammation of tonsils)in such condition of prolonged illness.
In view of prolonged fever with cough/cold and mildly raised polymorphs,I would like to get a CRP(blood test for bacterial infection)level and ASO(for streptococcal tonsilitis) titre done.

Continue giving the medication as advised and make a temperature chart(regularly at 8 am,2 pm,6pm and 10 pm;note down the timing if there is fever in between)to know the pattern of fever.
If fever is not high grade(more than 101/102 F) it would be better to use paracetamol(crocin/calpol)as an antipyratic rather than meftal.

If fever is less than 100 F and CRP/ASO comes normal, I won't get overly worried as sometimes it may take 3-4 weeks for complete resolution of cough/cold.

Fever pattern - It comes in every 4-6 hours. In most cases it close to 101. Some times, nose gets block due green thick mucus then start breathing through mouth. There is no headache. I have asthama and I used to carry asthalin inhaler and take 2-3 times in a day. My wife also has allergical problem. Some times, he vomets too.

Get him evaluated by an ENT specialist

Detailed Answer:
With history of such high grade fever for more than a week and presence of nasal discharge,cough and cold;I would like to get him evaluated by an ENT (ear,nose,throat)specialist to rule out sinusitis or tonsillitis. Get CRP test done.
He may also need an antibiotic course.
In presence of positive family history chances of getting allergic disorders are more in children.,but this does not seems to be only an allergic problem.

Please let me know about the further proceedings.
