What Do These Following Lab Reports Indicate?

Question: Hello Dr, my mom is suffering from diffuse disc bulge more towards left at l4-5 causing moderate impression on thecal sac and adjucent traversing exiting nerve root. This was shown in MRI.As a result she has severe right leg pain.she started medicinea such as retense, pregmin and sigmin b. 9days back but haven't started exercise. Suddenly last 2 days she haven't taken medicine and yesterday she took medicine but still she had pain in right leg. What should she do so that leg pain goes away?. Does surgery required? She gets pain more in the night while sleeping. Waiting for your reply. Advance Thanks a lot to u.have attached the reports. kindly suggest how big is the problem by seeing the report and what treatment she should take

Hello,
Thanks for trusting us with your health concern.
I have seen the attached reports. The disc bulge is mild and can be treated with the help of medicines. Medicines are the first line treatment along with physiotherapy. If the patient responds well with medicines then physiotherapy helps in preventing further progression of the problem. Surgery is kept as a last resort to improve the quality of life of the patient.
The main treatment of such problem is bed rest along with pain killers and muscle relaxants. Muscle relaxants and serratiopeptidase can help with her symptoms if used in appropriate dosage in combination with a potent analgesic. You can get these prescribed from her orthopedician along with the medicines she is taking. She needs to continue with medicines till she starts with regular physiotherapy.
Local analgesic gels and hot fomentation may provide a short term relief. She can use analgesic gels like Volini gel. Exercise is not advisable in the times of acute pain.
Using an LS contour belt of appropriate size might be helpful in providing support to back muscles.
Physiotherapy also plays a major role in treating such conditions.

Thanks a lot doctor for your response. But one quick question if her report shows mild, then how come she has intense pain whenever pain comes mostly in night. We showed to 4 orthopaedic doctors and none of them could fix her problem. They gave pain killers which would get her relief for 4 to 6 hours and once she stops the pain killer medicine she would get the pain again. Moreover, I noticed the pattern, she gets pain very severe in night while sleeping. Since none of the orthopedic could fix her problem, we saw neuro doctor. But as the pain has started again after some relief for few days after taking neuro medicine , I had lost hope on neuro doctor treatment and medicine too. Am not able to see her in intense pain during night. Yesterday, even after she had taken medicine prescribed by neuro doctor Retense and Prenig, she had severe pain in her leg and I had to give her Nimdis P, a pain killer suggested by orthopaedic doctor. Is this Pain killers only a way to treat her? How long she should be taking and isn't it harm? None of the doctors had suggested for physiotherapy. Neuro doctor suggested some exercises. I am worried and not sure which Treatment is going to get her full relief. Please help doctor.

Hello.
Thanks for writing again.

Even if the disc bulge is mild the pain caused by the mild nerve compression can be severe. Pain killers- Nimidis P - need to be given along with muscle relaxants like- Tolagin 8mg at least twice a day for pain relief. The improvement will happen only after lumbar traction- physiotherapy and supplements of methylcobalamin and pregabalin. it is best to continue with the medicine given by neurologist and get a muscle relaxant prescribed from orthopedician in addition to it. Once the pain subsides with medicines do start with physiotherapy for a persistent relief. Changing the doctors and leaving the treatment after every few days will not help. She can take pain killers round the clock for 3-4 weeks if needed.
