What Do These Lab Reports Indicate?

Question: The patient exercised according to the XXXXXXX Protocol for 2 m 49 s achieving a work level of XXXXXXX METS : 4.60.
Patient details :
Age 55 years, Female, Height 138 cm , Weight 50 kg, Diabetic, Having thyroid issues, allergic coughing at times.

Resting Heart rate initially 118 bpm, XXXXXXX to a XXXXXXX heart rate of 148 bpm ( 90% of Pr. MHR).
Resting blood pressure 100/60 mm Hg, XXXXXXX to a a XXXXXXX blood pressure of 140/80 mm Hg.
ST changes seen in inferior and lateral leads after exercise associated with pain in Lt arm and Jaw which was relieved after rest.
Test is positive for reversible myocardial ischemia.

NT - pro BNP using Immunofluorescence quantitative assay :1230 pg/mL.

Colour Doppler Echocardiography final impression :
Normal valves and chambers
LVEF 72±3% by 2D volume, single plane, apical 4-chamber view.
E/A = 0.6 (A > E) s/o diastolic dysfunction Grade 1.
Hs Troponin I : 84.10 ng/L

Please let me know what is the extent of risk involved in this case?

Hello

After going through the medical details provided by you I would like to tell you that considering her previous medical issues like Diabetes and other issues and current status she need urgent hospitalisation in a cardiac center with XXXXXXX lab facility.

Her Echo test is alright but her TMT test results as well as Troponin I levels are suggestive of myocardial injury probably a heart attack.

Once she feel little better urgent Angiography is needed to she the coronary arteries disease status.

Only after an angiography proper risk assessment can be done. As of now yes there is a moderate risk considering her current reports.

Thank you Sir.

The doctor currently supervising her condition suggested the same once she gains more stability. Presently, she was having a difficult time even for standing properly.
She is currently admitted in a hospital in XXXXXXX in XXXXXXX Any suggestions about where it would be better to see a cardiologist ( anywhere in XXXXXXX ) and what care should be exercised while taking her there for analysis and treatment?

I have gone through the TMT report attached by you and there are significant ST T changes suggestive of myocardial ischeamia.

Let her stabilise first, if the hospital in which she is admitted has a cardiologist and a XXXXXXX lab then at least get an angiography done so that we can have an idea of the situation. Later you can decide to take her to any good cardiac hosp like in XXXXXXX XXXXXXX or XXXXXXX

Sir, Thanks for your advice. My mother has been now admitted to Jeevak Heart Hospital and Research Centre in XXXXXXX under a cardiologist. Angiography has not been performed but right now while I am reporting, I guess preparation for CT scan etc. is being done. She is relatively more stable now. I shall provide you with further reports if needed.

Also, The aforementioned hospital should be okay for the time being. Isn't it so? Any suggestions.

If possible, I request you to suggest genuinely good doctors and hospitals where there is a hope to counter ILD. We are not sure if the doctor has diagnosed appropriately. He seems to have given up here.

ILD is ok but what about her heart, what they said about TMT result ??

ILD was not the primary issue. Have they done echocardiography on her if yes tell me the results.

For ILD XXXXXXX heart and lung institute is good. Although I don't have any personal reference there.

The strange thing for me is why they are planning or advising Angiography to a patient who was admitted with chest pain, TMT positive and breathing difficulty.

In my opinion you should take her to fortis or XXXXXXX hospital which ever is there is XXXXXXX and take a second cardiologist opinion.

Sir, Hello!
We consulted a few good doctors all over XXXXXXX and let them have the reports, they all concurred that the problem is fundamentally ILD - UIP and perhaps the oxygen deficiency detected as myocardial ischemia was a consequence of Interstitial Lung Disease reducing the amount of oxygen taken in and thereby carried by the blood.

After considering her age, diabetes and thyroid problem which all are risk factors to coronary artery disease together with such a TMT I would still like to suggest you to get an Angiography done to be 100% sure that there is no heart problem. Because that can later be a life threatening situation if left unattended.
