What Do These Lab Test Reports Indicate?

Question: Dear Sir,

I have given all details and symptoms of illness of my father. I have also attached the Xray report, Sonograpy report, blood test etc. Please consider and diagnose.
My father name: XXXXXXX xxxxxx
Age:72 years
Weight: 43 kg
Health history:
He has no operation anything and normal. But he was suffering from piles for last ten years and took treatment by injection on December 2015. Now it is cured. In very few times for last 3 years he passes urine only few drops. This is very rare case.The first sign of illness occurred on 3 April 2015.
Details:
On this date 3 April 2015 he suffered for first time with jaw pain, throat was tight but no sweating, sudden weakness, there is no pain in heart. My relative is a doctor. He made ECG of heart after 8 hours of this incident. I have attached the ECG file. The report was normal. After one month he suffered same type of pain again. This type of pain repeated again after two months of the first incident. This type of pain occurred four times.
He was suffering from piles for last ten years. From October 2015 he was suffering with weakness, loss of appetite. On December 2015 he was treated for piles by injection (one time only) with medicines such as tablets and syrup for constipation. Now the piles desease is cured. After few days of this treatment, he was suffering with very much weakness and loss of appetite. For last 1.5 months the body condition is very bad. He has difficulty in breathing whole night. Some times he breathe with mouth. Most of the time he don't sleep whole night. He has cough. There is a pain in stomach and other parts of the body such as knee joints, legs and back. Now the degree of weakness is that he is unable to walk for five minutes. He has vey much fatigue. Some times he is unable to speak for one minute, he tired very much. He is unable to withstand the pain and makes sound.
On 15 February 2016 I consulted other doctor for these symptoms. He made chest Xray, sonograpy, blood test and phlegm test. I have attached all files of these reports. His conclusion is that all reports are normal, therefore this is a psychiatric disorder. He has given one tonic and sleeping tablets. But I am not satisfied with his report.
Kindly inform the next step to be taken and diagnose.

I can understand your concern.
I have gone through the reports you have attached.
His chest x ray is showing bilateral pleural effusion with lower lobes collapse and consolidation.
This is common in heart related diseases.
So we should first rule out heart diseases in his case.
So get done 2 d echo first.
If required CT coronary angiography should also be done.
Please let me know
1. Has he ever undergone 2d echo?
2. Has he ever tested for serum protein level?

Dear Sir,
My father has not under gone 2d echo and no test made for serum protein level.

The Xray report shows problem with lungs. He becomes tired for walking short distances and also for talking for one minute. He has weakness and fatigue.

The sonography report shows problem with urine bladder and also for stomach. He has pain in stomach. Is there any infection?
What test to be taken for lungs functioning?

Thanks for your follow up question on Health Care Magic.
I can understand your concern.
No, stomach infection can not cause this. It will cause diarrhoea and vomiting.
His ultrasound report is saying cystitis. This means infection in Urinary bladder.
He is also having cardiomegaly and bilateral pleural effusion. So possibility of ongoing heart failure is more. And this can also cause tiredness, weakness, fatigue etc.
This is the reason why I am insisting on 2d echo.
To know the lung functions, you need to get done Pulmonary Function Test (PFT).
Please let me know if he is having burning micturation and decreased urine output.

Dear Sir,

He has no burning micturation and no decreased urine output.

On 3 April 2015 he suffered for first time with intense jaw pain, twisting of jaws, throat tightening, sudden weakness, but no sweating, no pain in chest. ECG was normal, done after 8 hours of this incident. This type of pain occurred four times in four months. What this shows?

The Xray photo shows fluid accumulation on both sides of chest. Has he any lungs infection such as pneumonia?

He has pain in back, knee joints and legs and sometimes difficulty in breathing in night. So don't sleep most of the time. He has very much weakness and very much loss of appetite. What this indicates?

For me, he is mostly having heart disease.

Thanks for your follow up question on Health Care Magic.
I can understand your concern.
For me, his symptoms are mostly due to heart disease.
His recurrent episodes are commonly seen in heart diseases.
Bilateral pleural effusion is commonly seen in heart failure. Pneumonia is unlikely in his case.
He is having weakness, not able to sleep, more symptomatic at night etc are suggestive of heart disease more.
So better to consult cardiologist and get done 2d echo and other cardiac tests.
Please let me know if ecg is done or not?
Diuretic drug given or not?
