What Do These Thyroid Profile And FNAC Test Report Indicate?

Question: I went through various tests and reports are as under for your advice please.
A).Thyroid Profile:-
T3 =1.49
T4 = 9.98
TSH=1.19

B).There is no past history of Tuberculosis in the family and chest x-ray(PA) view states:
Bony cage and Soft Tissues are normal, Trachea is normal, B/L CP angles are clear, The pulmonary vessels are normal, Roots as well as BV markings are accentuated, the heart size is normal. IMPRESSION; FINDINGS ARE SUGGESTIVE OF BRONCHITIS

C.) Have gone through FNAC test and report is as under on 2nd July 2015
F.N.A.C SWELLtNG NECK t LEFT SIDE)
Smear shows lymphoid cells in various stages of development,
including cenirocytes, centroblasts and immunob(asts. Thereis present
proliferation of reticulin fibres, histiocytes and scattered epitheleoid cells.
No malignant pathology seen.
OPINION : POSSIBILITIES: 1.REACTIVE LYMPHADENITIS, 2.? TUBERCULAR PATHOLOGY.
ADVISED REPEAT F.N.A C AFTER COURSE OF ANTIBIOTICS.

Further the report of FNAC done on 19/8/2015 is as under
F.N.A.C SWELLING NECK
The aspirate reveals brownish haemorrhagi fluid
which has few histiocytes and lymphomononuclear
cells.In addition is an occasional cell clump with
slight increase in NC ratio. they are cohesive
OPINION CYSTIC LESION ; NEEDS WORKUP TO RiO
DEPOSITS FROM THYROID

Possibility of tubercular lymphadenitis is more.

Thanks for your question on HCM.
I can understand your concern.
I have gone through the reports ,you have attached.
Possibility of tubercular lymphadenitis is more.
Better to get done either excision biopsy and histopathological examination or ultrasound guided FNAC.
Routine FNAC is blind procedure and so possibility of false negative results is high.
SO better to get done USG guided FNAC for proper tissue sampling or excision biopsy of the node.

Have gone through ultrasound guided FNAC and report is as under.
FNAC site: (a) Left Side Neck Nodule
(b) Right Thyroid nodule
Diagnosis:
ASPIRATE FROM LEFT NECK NODE SHOW BLOOD COMPONENTS ONLY.
ASPIRATE FROM RIGHT THYROID NODULE SHOW FEATURES OF COLLOID GOITRE.

DESCRIPTION:
(a): Smears show blood components only.
(b) : Smears show follicular cells in single and in groups exhibiting bland cytology. Background show cyst macrophages, few intact follicles are colloid.

Kindly advise on next course of treatment.

I can understand your concern.
Now you are having two options.
1. Get done excision biopsy and histopathological examination to rule out tuberculosis.
2. Take broad spectrum antibiotic fir 10 days and get done repeat ultrasound examination of neck to see whether lymphnodes are regressing or not.

Thank you doctor, I took the antibiotics after the first ultrasound and FNAC on 2nd July as prescribed by the doctor and thereafter no antibiotic till yesterday for ultrasound guided FNAC.
1.Will excision biopsy help to find out the cause of this and what could be the complications of this biopsy.
2. Will there be healing with medication for above problem in neck nodule, right thyroid
3. I am slightly scared as the earlier FNAC was done at small city and later in better labs and city. All reports are still not same and leading to fear growing day by day.
4. Is there a treatment for above symptoms, as one of the doctor advised to go for excision biopsy to find out. 1. tb, 2. Cancer 3 Thyroidectomy or removal of all lymph nodes depending on report in 20 minutes from the pathologist.
5. In case I go in for excision biopsy which may have different line of treatment based on the pathology report, how do I proceed and consult the doctor before hand so that all possibilities are shared and cleared before the operation.

No need to worry for excision biopsy. It is safe procedure.

I can understand your concern.
In my opinion, you should definitely get done excision biopsy. FNAC has its limitations. Even after ultrasound guidance, it is not 100% representative of whole lymphnode.
Only excision biopsy will be 100% diagnostic for your problem.
It will definitely tell you about tb, cancer or other infection.
And on the result of this, treatment is directed.
In my opinion, you should not worry about cancer.
And no need to worry for safety issue. Excision biopsy is safe and very short procedure.
