Question: a small hard lump (abut pea size) under only one nipple It is sore to the touch ( hurts her a bit)

Small swelling beneath the nipple in a 4 year old girl could be a Sebaceous cyst or an abscess.
I prefer to give Antibiotic and anti inflammatory tablets for similar complaints. If there is no response, ultrasound of the affected area should be done.

Antibiotic is a prescription based medicine. Antiinflammatory tablets are available over the counter. Children's tylenol is one such antiinflammatory drug which you can start using now. If symptoms don't improve, visit a doctor for an ultrasound scan and antibiotic prescription.
