Brief Answer:
nose surgery followed by ear surgery.

Detailed Answer:

Hi,

I had gone through your question and understand your concerns that you are having hearing loss.

As per CT scan, you require nasal surgery followed by ear surgery, because Eustachian tube function is first restored for successful outcome of ear surgery. (euschacian tube dysfunction occurs due to deviated septum and sinusitis.

Nose- septoplasty+ Turbincetomy + FESS.

Ear - tympanoplasty (tympanic membrane perforation) + modified radical mastoidectomy (soft tissue density (cholesteatatoma).

I suggest you to undergo surgery as soon as possible to avoid complications of cholesteatoma.
