The following findings are present in your recent CT scan lungs.
1. Right lower lobe 4.6 mm nodule
2. Inferior to above 2.5 mm nodule
3. 2 to 3 mm nodule in ?inferior right lobe and medial right lower lobe
4. 2 to 3 nodules in lingula
Imaging guidelines are in place and follow up imaging is recommended.
The Fleischner society (group formed to do research on imaging of lung nodules) recommends the following for nodules of size 4 to 6 mm on CT scan lucgs,
(i) In a person with low risk (Minimal or absent history of smoking and of other known risk factors): Follow-up CT at 12 mo; if unchanged, no further follow-up
(ii) In a person with high risk (History of smoking or of other known risk factors): Initial follow-up CT at 6–12 mo then at 18–24 mo if no change
It is important to know why you got the CT scan done and what are your symptoms. Since the nodules were not present in your last scan in 2006, it is best to follow up after 6 months and that is what your doctors have proposed.
The nodules are very small and require followup imaging to confirm they are benign. These can also be due to an infection and require complete clinical assessment by a pulmonary doctor and treatment after making a diagnosis.

Any supraclavicular (above collar bone) lump is a health concern unless it is confirmed to be benign. Since it is confirmed to be a lipoma in your case, it is no longer a concern.
In the absence of any symptoms, the next step is doing another CT scan after 6 months to know if the nodules are increasing in size. You can have a discussion with a pulmonary doctor on that, but in the absence of any symptoms, a wait and watch approach is followed in patients having CT scan lungs findings like the ones you have.
TSH levels need close dose adjustment so that there is no over treatment and to balance the levels of thyroids hormones.
